Creating a Healthier Sauk County—Together

Our Vision: Health for All

Everyone deserves to live in a healthy, thriving community. But how do we know what needs to change—and how to make it happen?

Every three years, Public Health Sauk County works with local partners* to take a deep look at the health of our community. This is called a Community Health Assessment, and it's how we find out what people need to live healthier lives.

* The Community Health Assessment is led by the Health & Wellness Steering Committee and the Sauk County Data Council. Public Health Sauk County facilitates these groups. The steering committee includes representatives from Reedsburg Area Medical Center, Sauk Prairie Healthcare, SSM Health St Clare - Baraboo, and the Ho-Chunk Nation-- all of whom create Community Health Improvement Plans for their agencies. Many additional organizations are part of the Sauk County Data Council, including local government leaders, school districts, law enforcement, and social service agencies.
