Applying For A Passport
The Sauk County Clerk’s Office accepts passport applications on behalf of the Department of State by Appointment ONLY.
To schedule an appointment: please contact us at (608) 355-3286. We are able to schedule appointments Monday through Friday. On the date of your appointment, please come to the West Square Building at 505 Broadway, Room #144, Baraboo.
We do provide photo service. The fee for passport photos is $12.00.
Fees for the Department of State will be sent in with the application and must be in the form of check or money order. One check or money order per application. County Clerks processing and photo fees may be cash or check.
