Community Health Improvement Plan

We all want to live in a healthy, thriving community—but how do we turn that vision into reality? In 2024, Public Health Sauk County teamed up with our three local hospitals and a range of community partners to find out. Through surveys, focus groups, and interviews, we listened to hundreds of voices across the county.

What we heard was clear: our community's top concerns remain mental health, housing, and substance use. And we’re not just listening—we’re taking action. We lead dedicated community action teams focused on these priorities. These teams meet regularly, bring together diverse perspectives, and are guided by the people most impacted by these issues. Together, we're digging into root causes and building practical, long-term solutions that create real change.
