** Please bring your child’s past immunization record with you
Immunizations are by appointment only. Please call 355-3290 to schedule – Clinics may be cancelled

VACCINES OFFERED BY THE SAUK COUNTY HEALTH DEPARTMENT

VACCINE FOR CHILDREN (VFC Eligible)
Available for all VFC eligible children under the age of 19 years and free of charge
1. DTaP - Diphtheria, Tetanus, and Pertussis
2. Hepatitis B
3. Hepatitis A
4. Polio
5. HIB- Haemophilus influenzae type b
6. Pneumococcal- Pneumonia
7. Rotavirus
8. MMR- Measles, Mumps, and Rubella
9. Varicella-Chickenpox
10. HPV- Human Papillomavirus for adolescents
11. Tdap-Tetanus, diphtheria, and pertussis for adolescents
12. Meningococcal-Meningitis for adolescents
13. Influenza- Flu vaccine for children older than 6months

ADULT VACCINES
Available to anyone over the age of 18. A fee for each vaccine may be charged.

1. Tdap- Tetanus, diphtheria, and pertussis
2. Td- Tetanus and diphtheria
3. Hepatitis A
4. Hepatitis B
5. Influenza- Flu vaccine
6. HPV- Human Papillomavirus for women up to age 26
7. Meningococcal-Meningitis Vaccine
8. MMR- Measles, Mumps, and Rubella
9. Varicella-Chickenpox vaccine

YOU MAY MAKE AN APPOINTMENT FOR IMMUNIZATIONS IN THE HEALTH DEPARTMENT OFFICE BY CALLING: 355-3290.
