Community Health Needs Assessment
We want to hear from you!
Sauk County Health Department along with our partners Reedsburg Area Medical Center, St. Clare Hospital & Dean, Baraboo and the Sauk Prairie Health Systems are launching an assessment of the health of our communities. There will be several opportunities to participate in the assessment. All participating partners will post a survey to complete on their websites, Facebook and Twitter accounts. It can also be accessed at https://docs.google.com/forms/d/1nisziwCSFW6AAyRtFRuiovwFpZsdsj4QnbSMHk_uV9s/viewform?usp=send_form
The survey will be available from July 6 through July 31. It only takes about 10 minutes of your time to complete.
You will be eligible to win a prize for completing the survey. Prizes include FitBits, iPads and various Dells attraction tickets.
The Health Department will be holding listening session throughout the county to get feedback. Contact the Health Department at 608-355-3290 if you are interested in attending a listening session.
