PHSC Community Health Improvement Plan

We all want to live in a healthy community

How should we focus our efforts? How can we best improve? Public Health Sauk County worked with our 3 area hospitals and other partners to ask our community these questions in 2021 via surveys, focus groups, and interviews.

What we heard: top concerns are mental health, housing and substance use.

We formed teams of community members to act on these issues, making sure those who are affected by these problems are part of the solution. Read on to learn about our plans for improvement, and let us know what you think: do you agree with our focus, and what more can we do?

Housing:

Where you live has a major impact on your physical and mental health and well-being. All people deserve to live in decent, stable homes at prices they can afford.

Mental Health:

Good mental health enables us to be our best selves, to cope with stress, work effectively, develop relationships, and make a contribution to our community. Barriers to mental health care make it difficult for everyone to achieve their best mental health.

Substance Use: Prevention

Substance use and addiction have become major concerns in Sauk County. Young people who use vapes, alcohol, and other drugs are at higher risk for developing addiction later in life.

Substance Use: Response

Our response to substance use includes increasing access to treatment to improve health and wellbeing and offering harm reduction: tools, strategies, and ideas that reduce the negative consequences associated with drug use, such as overdose.
