The Sauk County Community Care Program, nicknamed "the Voucher Program," provides acute health care services for individuals without health or dental insurance.

The program was developed through the cooperation of local health care providers and is designed to meet the health care needs of Sauk County residents who are uninsured and are income eligible at below 200% poverty level. The program is unable to provide services for ongoing chronic health conditions, inpatient hospitalizations, medical specialists, surgery, physical therapy, occupational therapy, speech therapy, or continual prescription help.

 

Services Provided:

  1. Medical Voucher: for acute care needs such as upper respiratory infections, sexually transmitted infections and acute pain or injury issues. No vouchers for chronic care management.
     
  2. Dental Voucher: for acute dental needs. The extent of the service provided is determined by the individual dentist providing care.
     
  3. Vision/Glasses Voucher: for those without vision coverage who need an eye exam and/or glasses.
     
  4. Prescription Voucher: for birth control or for a one-time 30-day supply of generic or low-cost medication. Narcotics and controlled substances are not covered.

 

How Do I Get a Voucher?

  1. Bring proof of residency and income to the Public Health Department to determine eligibility. Hours are 8:00am to 12:00pm Mondays through Fridays. If you are feeling sick, please call 608-355-3290.
     
  2. Meet with the Public Health Nurse to determine medical/dental/vision need. A voucher may then be written for the appointment or prescription need.
     
  3. Appointments can only be made with participating health care providers.
     
  4. Medications can be provided on a one-time 30-day supply basis and can only be filled by Corner Drug in Baraboo and RAMC Community Pharmacy in Reedsburg. Narcotics and controlled substances are not covered. Baraboo area residents may also be asked to pick up an additional prescription voucher at St Vincent De Paul in Baraboo.
     
  5. Individuals with a voucher must present the voucher to the medical provider at the time of service. Vouchers cannot be backdated.
     
  6. The voucher only covers care for the problem identified on the voucher.
