SIGNED FINAL 1-5-2022_MOU BETWEEN THE STATE OF WI DEPART OF AG, TRADE AND CONSUMER PROTECTION AND SAUK COUNTY HEALTH DEPART 12-20-21

THIS MEMORANDUM OF AGREEMENT (“MOA”) is made and entered into for the period 07/01/2022 through 06/30/2025 (“Performance Period”) by and between the Wisconsin Department of Agriculture, Trade and Consumer Protection (“DATCP”), whose principal business address is 2811 Agriculture Dr., P.O. Box 8911, Madison, WI 53708-8911 and SAUK COUNTY HEALTH DEPARTMENT (“Agent”), whose service address is 505 Broadway, Suite 372, Baraboo, WI 53913, for the purpose of addressing issues of mutual interest to the parties regarding Wis. Stat. §§ 97.41 and 97.615 and Wis. Admin. Code ch. ATCP 74.  The Agent's jurisdiction under this MOA includes the following geographic area(s): Sauk County.   The Department shall issue a future MOA to the Agent by January 1, of the last fiscal year of the current MOA. The Agent shall commit to continue as the Department’s Agent for the future MOA period, by signing and returning the MOA by March 1, of the last fiscal year of the current MOA.
