This is a renewal of a 10 year contract we have via Adams County and Sauk County Health departments to the fulfill WDCF Contract No. 437003-G16-0000701-R04-01, a copy of which is hereby incorporated by reference (the “WDCF Contract”), under which grant funds are dispersed to Grantee for Family Foundations Comprehensive Home Visitation Program, and Grantee will in term disperse funds to Sauk County Health Department. Sauk County Health Department shall be a Subrecipient of Adams County Health & Human Services. (Year 6 of 10)
