The meeting was called to order at 5:33 p.m. by Stehling.Certification of the open meeting law was noted.
Motion by Miller to approve the November 13, 2013 Public Health Board Agenda, second by Carlson. Motion carried.
Motion by Nobs to adopt the minutes of the October 21, 2013 Public Health Board Meeting, second by DeLong.Motion carried.
Communications: Bodendein had emailed: an email from Dan J Hopfensperger at DHS dated 11/05/13 regarding Pertussis Update, Did You Know – CDC – Preterm birth and pregnancy outcomes dated 11/04/13, Cathy Warwick handed out a letter from the National Association for Home Care & Hospice, regarding “Home is the Center of Health Care.”

Business Items for Discussion and/or Action:
- Closed session Performance evaluation of the Public Health Director/Health officer. Motion by DeLong, second by Carlson to enter into closed session pursuant to Wis. Stats. 19.85 (1) (c) considering employment, promotion compensation or performance evaluation data of any public employee over which the governmental body has jurisdiction or exercises responsibility. Role call vote was taken: Stehling – Aye, Nobs – Aye, Miller– Aye, Carlson– Aye, Delong – Aye. Motion carried.
- Motion by DeLong to reconvene into open session. Second by Carlson. Motion carried.
- Discussed Nurse Family Partnership. Warwick handed out packets of information for review. It was decided that the packet will be discussed further at the December Board of Health meeting.
- Accreditation, Warwick presented a power point for Domain #2. Discussion followed.
- Warwick reported that Sauk County had an Enrollment Fair for the Affordable Care Act on October 29, 2013. 44 people attended, 27 filled out an exit survey. 16 people wanted more meetings in Baraboo, 8 people wanted more meetings in Sauk City area and 3 people wanted more meetings in Reedsburg. There are two more enrollment fairs scheduled, on November 26, 2013 from 10:00 a.m. to 7:00 p.m., and December 12, 2013 from 9:00 a.m. to 4:00 p.m., in Baraboo.
- Carlson reported on Community Health Improvement Plan. At their meeting they had a discussion regarding the mental health professional shortage. They want to make sure we “need” another mental health professional. They want to make sure we know the problem before we try to solve the problem. Local hospitals are talking to the medical community about behavior health professional. Carlson reported that the Sauk Prairie hospital is working on a database, for smaller communities for better health, with the Population Health Institute.

- Reviewed the monthly financial reports for October 2013.
- Motion to approve the October 2013 invoices of $24,786.19 by Nobs, second by Miller. Motion carried.
- WPHA/WALHDOB – Stehling reported on WALHDAB.

Next meeting date: Discussed changing the Board of Health meeting dates to the second Monday of each month. The next meeting date will be, December 9, 2013, at 5:30 p.m.
Motion by DeLong, second by Carlson to adjourn the meeting. Motion carried. The time was 6:41 p.m.
