Purpose

To publish the current fee schedule for the programs offered through the Aging & Disability Resource Center of Sauk County.

Background

This is the third version of the fee schedule published in 2014 due to the restructuring of ADRC programs.  This version incorporates the shopping/grocery bus fare of $3.00 (round trip per person), effective October 1, 2014.

ADOPTING PROGRAM FEES FOR THE

AGING & DISABILITY RESOURCE CENTER OF SAUK COUNTY     

            WHEREAS, the Aging & Disability Resource Center Committee has established reasonable program fees for the AddLIFE Today! News Magazine, Congregate Dining Center Lunches, Home Delivered Lunches, Taxi Subsidy Punch Cards “Get Out and Go,” a Demand-Response Bus System Including One-day Excursions “Fun-day Travels” and Grocery/Shopping, and Volunteer Driver Escort Rides operated through the Aging & Disability Resource Center of Sauk County; and,

            WHEREAS, the fees set forth are in accordance with United States Federal Code, State of Wisconsin Statutes, and County Ordinances and Policies, and;

            WHEREAS, a listing of these fees is as follows.

           Now, Therefore, Be It resolved, by the Sauk County Board of Supervisors, met in regular session, that the fees schedule for the Aging & Disability Resource Center of Sauk County be and hereby is adopted, to be reviewed annually by the Aging & Disability Resource Center Committee, until such time as the fees are in need of change.

For consideration by the Sauk County Board of Supervisors on Tuesday, September 16, 2014.
