Purpose

Dr. DeLong resigned her position, because she was appointed to the Board of the Association of American Indian Physicians (AAIP).  This Board will take up more of her time and she won't be able to fulfill the duties on the Board of Health.

Background

It is the custom of the Board of Supervisors to recognize individuals who have served the people of Sauk County with distinction.  Dr. Amy DeLong has faithfully served as a Community member on the Board of Health for more than eleven (11) years.  Dr. Amy DeLong resigned her position on the Board of Health, on August 14, 2018.

RESOLUTION HONORING DR. AMY DELONG

 

Background:

It is the custom of the Board of Supervisors to recognize individuals who have served the people of Sauk County with distinction.  Dr. Amy DeLong has faithfully served as a Community member on the Board of Health for more than eleven (11) years.  Dr. Amy DeLong resigned her position on the Board of Health, on August 14, 2018.

NOW, THEREFORE, BE IT RESOLVED, by the Sauk County Board of Supervisors,

met in regular session, that the Sauk County Board of Supervisors expresses its sincere appreciation and admiration for Dr. Amy DeLong for 11 faithful years of service to the people of Sauk County;

 

AND, BE IT FURTHER RESOLVED, that the Chairman of the Sauk County Board of Supervisors is hereby directed on behalf of the Sauk County Board of Supervisors to present Dr. Amy DeLong with an appropriate certificate and commendation to express our highest esteem for her extraordinary contributions to our community.
