Application for Permit (Driveway/Culvert) on County Trunk Highway

Important information regarding the application for a driveway permit:

Only one driveway shall be allowed per parcel.

On a general location map mark proposed location. A plat book map may be used for this. Contact the Highway Department office at 608-355-4855 to request a copy.

For any new driveway or driveway to be moved, place flags or markers in the highway ditch or foreslopes at the proposed location.
