The Sauk County Health Department is expanding its Prenatal Care Coordination program to include Nurse Family partnership.

The Sauk County Health Department is expanding its Prenatal Care Coordination program to include Nurse Family partnership.  Nurse-Family Partnership Program is a voluntary program that started 30 years ago in Colorado.  Nurses do home visits starting as early as possible during pregnancy and continuing through the child's second birthday.

The nurses visit the women (families) on a weekly, bi-weekly or monthly basis over the 2½-year program cycle.
Sauk County Nurse-Family Partnership is a program for women who are having their first baby. If you enroll, a specially trained nurse will visit you in your home throughout pregnancy and continue to visit until your baby is 2 years old.
NFP nurses visits have shown measurable change resulting in: 79% fewer preterm deliveries, 50% reduction in child language delays, and 48% reduction in child abuse and neglect.

Any woman who is pregnant with her first child, less than 28 weeks pregnant (7 months), low income, and lives in Sauk County is eligible.
