Services to be Provided
The WDCF awarded grant funds shall be used to serve new families or families already enrolled in services that are not currently funded by Family Foundations Home Visiting. Services and responsibilities of Sub-Recipient are as follows:
A. Continued implementation of the Nurse Family Partnership (NFP) model, with demonstrated model fidelity.
B. Participation in benchmark reporting, continued quality improvement, and training requirements in accordance with the requirements of the WDCF Contract, including but not limited to any MIECHV funding stipulations therein.
C. Sub-Recipient will use the 2025-2026 funding to support existing home visiting staff and program infrastructure. These funds will also support equipment costs and NFP Program Support fees, as set forth in the Budget in Exhibit 2 of the WDCF Contract.

Payment for Services:
The total amount to be paid to the Sub-Recipient by the Grantee for standard programs provided under this contract shall not exceed $623,941 (six hundred twenty-three thousand nine hundred forty-one dollars). The Sub-Recipient is responsible for the budget match allocation of $394,657 (three hundred ninety-four thousand six hundred fifty-seven dollars).
Signatures
A. This Contract is agreed upon and approved by the authorized representatives of the Adams County Health & Human Services Department and Sauk County Health Department as indicated below.
B. This contract becomes null and void if the time between the purchasers’ authorized representative signature and the Provider’s authorized representative signature on this contract exceeds 60 days.
