On behalf of the SSM Health St. Clare Foundation and its Board of Directors, I am pleased to enclose a
check for $2,000 for Support People and Empower Recovery Coalition to help make Sauk County more recovery
friendly. This check represents your spring 2024 Healthy Community Partnership Grant from the St.
Clare Foundation and is funded through a special fund designated to assist those attempting to recover
from alcohol and drug dependency.
A signed copy of the fully executed Grant Agreement is enclosed for your records. This grant covers
