SERVICES AGREEMENT Sauk County Health Department SSM Health St. Clare Hospital - Baraboo This SERVICES AGREEMENT (“Agreement”) is effective __(last date of signature)___________, 2024 (“Effective Date”), by and between Sauk County (“PARTY”) and SSM Health Care of Wisconsin, Inc., a Missouri nonprofit corporation, doing business as SSM Health St. Clare Hospital - Baraboo (“SSM”).
WHEREAS, SSM desires that PARTY provide certain services as described below; and
WHEREAS, PARTY desires to provide services in accordance with the terms of this Agreement.
THEREFORE, it is understood and agreed upon by the parties hereto as follows:
1.Services.
PARTY shall provide all of the services set forth in Exhibit A, attached hereto and incorporated byits reference ("Services"). SSM and PARTY may add to or modify the Services by mutual writtenagreement at any time during the term of this Agreement.
