SIGNED FINAL-Sauk County Recovery Coach Community Engagement MOU Sauk Healthcare & WISHOPE & BAA 5-15-2026

PURPOSE:
This Memorandum of Understanding, ("MOU"), is executed by and between Entity A: Sauk County, Entity 
B: Sauk Prairie Healthcare, and Entity C: WisHope to facilitate a collaborative program focused on 
community engagement.

IMPLEMENTATION OF AGREEMENT:
Entity A (Sauk County) Agrees to:
1.  Assist Entity B and Entity C with patients who have substance use concerns in an effort to 
streamline a connection to treatment and other vital support services.
2.  Provide information to Entity B and Entity C on patients who have authorized Entity B to share 
their information with Entity A and C (Clients) to facilitate outreach and program administration 
goals. Client information to include: location, health and wellbeing, and client's perceived needs.
3. Meet as needed with Entity Band Entity C to review referral protocols and determine potential 
next steps.
4. Review with Entity B and Entity C Patient Outcomes for each mutual Client engaged in WisHope 
Peer Recovery Coach services.
5.  Reimburse Entity C for staff time as detailed in Services Contract between Entity A (Sauk 
County) and Entity C (WisHope).
