This Service Agreement (the “Agreement”) is entered into this 3rd day of June 2024 (the “Effective Date”) by and among Sauk County Health Department a County Health Department located at 505 Broadway St. Suite 372 Baraboo WI 53913 (“Client”), and Televox, Inc., a Delaware corporation (“Televox) located at 11650 Miracle Hills Drive, Omaha, NE 68154, and Pfizer Inc., located at 66 Hudson Boulevard East, New York, NY 10001-2192 (“Sponsor”). Client, Televox, and Sponsor are collectively referred to as the “Parties” and individual referred to as a “Party”).
