SIGNED FINAL MOU 468 Sauk County Public Health UW Health CL Site Agreement 3-2-2026

An agreement we have in order to be able to view medical records strictly related to our work.

The University of Wisconsin Hospitals and Clinics Authority (“UWHC”), the University of
Wisconsin Medical Foundation, Inc. (“UWMF”) and SwedishAmerican Hospital (“SAH”) jointly
provide access to their electronic medical record (EMR) information platform through UW
Health’s Care Link system and are referred to collectively in this Agreement as “UW Health”
jointly provide access to their electronic medical record (EMR) information platform through
EpicCare Link software (referred to in this Agreement as “Care Link”).
In addition, the UW Health EMR consists of medical records from UWHC, UWMF, SAH, as well
as UW Health Affiliates and our Community Connect members. The terms of this agreement
shall apply to all protected health information (“PHI”) maintained within or accessed using the UW Health EMR.
