SIGNED FINAL Edgewood University Affiliation Agreement for Sauk County Health Dept.prelim 2-13-2026

This agreement to affiliate in the education of students is made this 12th day of February
2026 between Sauk County Health Department (hereinafter called the Agency) and
Henry Predolin College of Health Sciences at Edgewood University (hereinafter called the
Affiliating Institution).
PURPOSE OF THE AGREEMENT
A. The Affiliating Institution has established programs in Nursing and Health Sciences
which require the patient care and patient service facilities of the Hospital for
practical learning and clinical experience.
B. The Agency has clinical facilities suitable for the educational needs of the Affiliating
Institution’s programs in Nursing and Health Sciences.
C. It is to the mutual benefit of both the Agency and the Affiliating Institution that
students have opportunities for practical learning and clinical experience as
students and as future health care workers.
