SIGNED FINAL Amendment to Practitioner Provider Agreement 10-8-2025

AMENDMENT TO PRACTITIONER PROVIDER AGREEMENT

The Practitioner Provider Agreement made and entered into by and among Dean Health Systems, Inc., a 
Wisconsin corporation ("Dean"), SSM Health Care of Wisconsin, Inc. ("SSMWI") and Affiliates and 
Sauk County Department of Human Services, a Wisconsin government agency ("Provider") effective on 
June 15, 2020, shall be amended as set forth below. Provider acknowledges that whenever Dean Health 
Insurance, Inc. ("DHI") and Dean Health Plan, Inc. ("DHP") agree to perform in a specified manner 
herein, it is understood that Dean and SSMWI (together "Obligors") represent and warrant that 
DHI/DHP shall perform in such a manner.
Effective October 1, 2025:

1. The Preamble shall be deleted in its entirety and replaced with the following:

This Practitioner Provider Agreement ("Agreement") is made and entered into by and among Dean 
Health Systems, Inc., a Wisconsin corporation ("Dean"), SSM Health Care of Wisconsin, Inc. 
("SSMWI") and Affiliates and County of Sauk, a Government agency ("Provider"). Provider 
acknowledges that whenever Dean Health Insurance, Inc. ("DHI") and Dean Health Plan, Inc. ("DHP") 
agree to perform in a specified manner herein, it is understood that Dean and SSMWI (together 
"Obligors") represent and warrant that DHI/DHP shall perform in such a manner.
