AMENDMENT TO
MEDICARE PRACTITIONER PROVIDER AGREEMENT

The Medicare Practitioner Provider Agreement entered into by and between Dean Health Systems, Inc., 
a Wisconsin stock insurance corporation ("Dean"), and Sauk County Department of Human Services, a 
Wisconsin government agency ("Provider") effective on June 15, 2020, amended effective on January 
2, 2009, and amended effective on June 15, 2020, shall be further amended as set forth below.

Effective October 1, 2025:

I. The Preamble shall be deleted in its entirety and replaced with the following:

This Medicare Practitioner Provider Agreement is made and entered into by and between Dean Health 
Plan, Inc., a Wisconsin stock insurance corporation ("DHP") and County of Sauk, a Wisconsin 
government agency ("Provider").

2. Section 8 MISCELLANEOUS shall be amended as stated below:

a. Subsection 8.6 Notice shall be deleted in its entirety and replaced with the following

8.6 Notice. Any notice required to be given pursuant to the terms and provisions of this Agreement 
shall be in writing and hand-delivered or sent by certified and prepaid mail, return receipt 
requested, addressed as follows:
