PROGRAM MEMORANDUM

BETWEEN

Sauk County, Wisconsin (the “CHIP Host Site”),

WISCONSIN AHEC, AND REGIONAL AHEC

 

 

This Agreement is entered into by and between the Board of Regents of the University of Wisconsin System on behalf of the Wisconsin Area Health Education Center (“Wisconsin AHEC”) with an address of 750 Highland Avenue, Madison, WI 53705, the South Central Area Health Education Center (“Regional AHEC”), with an address of 136 W. Grand Avenue, Suite 100, Beloit, WI 53511, Sauk County Health Department (“CHIP Host Site”), with an address of 505 Broadway, Suite 372, Baraboo, WI 53913

 

  1. Purpose of Agreement

 

WHEREAS, Wisconsin AHEC creates relationships among local public health departments, community health centers, and community organizations, student interns, and the Wisconsin AHEC Program. The success of the program depends on parties to this contract having both an appreciation for the roles they play in the relationship and a commitment to collaborative learning.

 

WHEREAS, many Students participating in the Community Health Internship Program (“CHIP”)  benefit from practical learning experiences (“PLE”) in real-world settings.

 

WHEREAS, CHIP Host Site desires to work with Wisconsin AHEC and Regional AHEC to provide PLE for Student Interns (“Student Intern”).

 

  1. General Information

 

  1. This Menorandum shall constitute a legally binding contract between the parties.  This particular contract is only between the Wisconsin AHEC, Regional AHEC, and CHIP Host Site.  Throughout this memorandum, the words “memorandum” and “contract” are used interchangeably. 

 

  1. It is agreed by the parties that good and valuable consideration exists for this contract.

 

  1. The PLE, which shall be supervised by the Sauk County Health Department, shall cover a period of time, as arranged between the Wisconsin AHEC, Regional AHEC, and the CHIP Host Site, within the time frame of June 5 - July 28, 2023. Such beginning dates and ending dates constitute the term of this contract.  CHIP Host Site shall be free to terminate this contract at any time, without cause, upon 15 days’ notice.

 

  1. The PLE shall complement the service and activities of the CHIP Host Site; however, it is understood that the Student Intern shall not be used in lieu of professional or staff personnel.

 

  1. The goals of the PLE shall be made clear to all persons involved, developed collaboratively between the Wisconsin AHEC, Regional AHEC, student, and the CHIP Host Site, and stated in the weekly action plan.
