Subaward Agreement

This Subaward Agreement (“Agreement”) is made and effective as of July 1, 2023, by and between 
Children’s Hospital of Wisconsin, Inc., a Wisconsin non-profit corporation d/b/a “Children’s Health 
Alliance of Wisconsin” (hereinafter referred to as “Children’s”) and

Sauk County
name and hereinafter referred to as “Subrecipient”).

BACKGROUND
Children’s has received an award from the State of Wisconsin Department of Health
Services, Division of Public Health for the Wisconsin Seal-A-Smile program (“the Project”). As 
specified in the Grant Agreement between the State of Wisconsin Department of Health Services and 
Children’s for Oral Health Program (the “Grant Agreement”), Children’s desires to subaward
a portion of the Oral Health Program award to Subrecipient.
