SIGNED FINAL 4-16-2024 RAMC-Reedsburg EHR Systems Access Agreement Between Unity Point Health and Sauk County Health Department

This System Access Agreement (“Agreement”) is effective as of the date last signed below, (“Effective Date”) between Iowa Health System d/b/a UnityPoint Health, an Iowa nonprofit corporation, on behalf of itself and the UnityPoint Health Affiliated Covered Entity (collectively “UnityPoint Health”), and Sauk County Health Department (“Participant”) (individually a “Party” and collectively the “Parties”).
