MASTER SERVICES agreement

This Master Services Agreement (this “Agreement”) is entered into, as of the date of the later signature indicated below (the “Effective Date”), by and between Public Health Sauk County, a local public health department in Wisconsin, with an office located at 505 Broadway St #372, Baraboo, WI 53913 (the “Client”) and Flourish and Thrive Labs LLC, an Illinois Limited Liability Company, with an office located at 140 Mallard Court, Grayslake, IL 60030 (“F&T Labs”). The Client and F&T Labs may be collectively referred to as “the Parties” or individually referred to as “a Party.”

WHEREAS, the Client desires to engage F&T Labs to provide certain services, and

WHEREAS, F&T Labs desires to provide those services to the client, upon the terms and subject to the conditions set forth below, and

WHEREAS, F&T Labs possesses certain knowledge, skills, and expertise that qualifies it to provide those services.

NOW, THEREFORE for good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, the parties hereto agree as follows:
