MetaStar_HRSA Grant - Sauk County Public Health Subrecipient 8-12-2022

Here is the explanation for this agreement: MetaStar is the prime recipient of a HRSA grant (Aug 2021-Aug 2024), and Sauk County public health is a subrecipient. LSS was a subrecipient of this grant under MetaStar as well, but last month MetaStar realized that LSS could no longer be their subrecipient, as there is a conflict of interest (under a separate DHS contract, MetaStar evaluates LSS for work not related to this HRSA grant). MetaStar asked if public health would be willing to receive a bigger subaward, and then subcontract with LSS for those additional services. That is the subaward agreement now under review. We apologize for the short turnaround time needed for this review. MetaStar’s contract with LSS ends today, 8/12, and we were waiting to hear back from the LSS legal team (they still have not yet responded with their review).

Amount: $260,000. Term: one year

THIS SUBAWARD AGREEMENT (“Agreement”) is entered into as of August 13, 2022, by and between Sauk County Health Department (“Recipient”), and Lutheran Social Services of Wisconsin and Upper Michigan, Inc., a Wisconsin corporation (“Subrecipient”). This Agreement shall govern certain activities and responsibilities to be carried out by Subrecipient on behalf of Recipient. Recipient is a subaward grantee of the U.S. Department of Health and Human Services Health Resources and Services Administration (“HRSA”). Recipient and Subrecipient may be referred to individually as a “Party” and collectively as “the Parties”.
