This amends the original 2021 OAA Contract agreement for the period of January 1, 2021 – December 31, 2021 by and between the "GRANTOR" Greater Wisconsin Agency on Aging Resources (GWAAR), Inc.,  and the "GRANTEE", ADRC of Sauk County.

This amendment will serve as notification that the GRANTOR is adding the amount of a new grant allocation by the amounts shown in the table below, in column "Contract Change Amount" and making the total payment to the GRANTEE in the amounts also shown in the table below, in column "New Contract Amount". the new contract amount is $33,751.00

All other terms and conditions of the original agreement remain unchanged.
