In Sauk County, three high schools completed the WI DPI 2019 Youth Risk Behavior Survey (YRBS). Public Health would like to aggregate the 2019 YRBS data we have collected from the three high schools, and to do so accurately, is seeking demographic survey data from DPI on these 3 schools to complement the survey data that the schools provided. For example: how many students/males/females answered each question's answer options. DPI requests this Data Use Agreement be signed by us before they release the data.
