Amendment to Food Service Agreement - dated October 5th, 2020

THIS AGREEMENT is entered into this 15th day of November 2021 by and between Sauk County, West Square Building, 505 Broadway, Baraboo, WI 53913 (hereinafter referred to as “Client”) and Consolidated Correctional Foodservice, a division of Consolidated Management Company, 2670 106th Street, Suite 140, Des Moines, IA 50322, (hereinafter referred to as “Consolidated”).
WHEREAS, Consolidated operates the food service for the Sauk County Jail facility located at 1300 Lange Court, Baraboo, WI 53913 (the “Facility”); and
WHEREAS, Client wishes to retain Consolidated to provide senior meal food service for ADRC of Eagle Country Sauk County- Baraboo Office, and Consolidated is willing to undertake such responsibilities, on the terms and subject to the conditions set forth herein.
• Consolidated will provide approximately 145 hot lunch meals Monday-Friday (exact meal count to be provided by ), except for County recognized holidays and days when the Baraboo School District is closed due to inclement weather. Client will notify Consolidated no later than 7AM if the School District is closed for inclement weather.
• Consolidated will invoice the Client $3.50 per meal on a weekly basis. Client shall pay Consolidated within fifteen (15) days after receipt of the monthly statement.
• The senior meal provided is the same meal as prepared for trustees at the Facility. Should senior meal requirements necessitate a change from the trustee meals Client and Consolidated shall create and agree upon a new menu.

The initial term of this Agreement (the “Initial Term”) shall begin on November 29, 2021 and shall continue until May 27, 2022. It may be terminated by notice in writing given by either party hereto to the other, at least 30 days prior to the date of termination.
