Addendum to Food Service Agreement Dated October 5th, 2020

THIS AGREEMENT is entered into this 15th day of May 2022 by and between Sauk County, West Square Building, 505 Broadway, Baraboo, WI 53913 (hereinafter referred to as “Client”) and Consolidated Correctional Foodservice, a division of Consolidated Management Company, 2670 106th Street, Suite 140, Des Moines, IA 50322, (hereinafter referred to as “Consolidated”).
WHEREAS, Consolidated operates the food service for the Sauk County Jail facility located at 1300 Lange Court, Baraboo, WI 53913 (the “Facility”); and
WHEREAS, Client wishes to retain Consolidated to provide senior meal food service for ADRC of Eagle Country Sauk County- Baraboo Office, and Consolidated is willing to undertake such responsibilities, on the terms and subject to the conditions set forth herein.
• Consolidated will provide approximately 145 hot lunch meals Monday-Friday (exact meal count to be provided by __________), except for County recognized holidays and days when the Baraboo School District is closed due to inclement weather. Client will notify Consolidated no later than 7AM if the School District is closed for inclement weather.
• Consolidated will invoice the Client $5.00 per meal on a weekly basis. Client shall pay Consolidated within fifteen (15) days after receipt of the monthly statement.
