On June 25, 2022 at approximately 7:05 AM, a deputy from the Sauk County Sheriff’s Department was dispatched to a report that a vehicle had struck a sign at the intersection of Grandview Ave and W. Grandview Ave in the town of Merrimac. The deputy made contact with the operator who was identified as Jeffrey A. Kowalchuk, age 58 of rural Wautoma.
While speaking to the operator, the deputy observed signs of impairment. Kowalchuk was asked to do field sobriety tests and as a result of those tests, Kowalchuk was arrested for Operating While Intoxicated - 4th offense, Operating after Revocation, and Fail to Install Ignition Interlock Device. Kowalchuk was transported to the Sauk County Jail.
