Dear Rebecca,
It is my pleasure to inform you that you have been named the recipient of a 2025 Healthy Life 
Initiative grant. This year there were 38 grant requests totaling $81,261 competing for the $40,000 
available funding. As always, it was a difficult selection process.

The grant committee is awarding Rural Safety Days funding in the amount of $2,500. Sauk Prairie 
Healthcare Foundation is happy to be able to assist your efforts for the betterment of our 
communities' health and quality of life.

You will find enclosed the Healthy Life Initiative contract, the grant disbursement and mid-year 
report document (referred to as Addendum A). Please sign the enclosed contract and return the 
signature page to Sauk Prairie Healthcare Foundation at 260 26th Street, Prairie du Sac, WI 53578 
or email to anne.brickl@saukprairiehealthcare.org by December 30, 2024.

We would appreciate receiving advance notification if and when program events are going to take 
place. Our committee members may appreciate the opportunity to observe, and we may ask permission 
to take photographs of our grant funded activities.
Your progress report(s) should be submitted via e-mail. Photographs tell the story, and are always 
appreciated along with any measurable outcomes such as number of participants. We must receive at 
least one report, the timing of which is dependent on the nature and timing of your project no 
later than August 31, 2025 to allow committee members to review progress reports prior to making 
2026 funding decisions. Organizations who do not submit reports will not be considered for future 
funding.

Photos, news releases and any print materials from your project are great ways to demonstrate how 
the grant funds are being used. You may send these materials in digital format to me at 
todd.wuerqer@saukprairiehealthcare.org.Of course, mailed material is acceptable too.
