Health Education & Prevention Agreement

The Health Education and Prevention agreement (hereinafter referred to as the "Agreement") is 
entered into as of August 22, 2022 by and between School District of Baraboo (hereinafter referred 
to as the "Client") and Sauk County Health Department, (hereinafter referred to as the 
"Department"), collectively referred to as the "Parties", both of whom agree to be bound by this 
Agreement.

1. Purpose. The Client contracts with the Department to provide health education and prevention 
related services including but not limited to:
a. Parent education events relating to substance use and prevention topics
b. Parent education events relating to youth mental health promotion
c. Student education events or curriculums relating to substance use protective factors and/or risk 
factors offered during instructional time or afterschool settings
d. Student leadership development activities including but not limited to: supporting youth 
leadership council focused on substance use prevention in Sauk County youth ages
12-18 years old.
e. Facilitating youth focus groups for assessment purposes.

2. Agreement length. The agreement will be in effect from the date of this agreement until August
l'ᵗ, 2023.

3. Financial Support. No compensation will be provided.

4. Cancellation. This Agreement may be terminated at any time by either Party upon 30 days written 
notice to the other party.

5. Evaluation Data Collection. The Department may record evaluation data of the services provided 
including but not limited to
a. Parent education events relating to substance use and prevention topics
b. Parent education events relating to youth mental health promotion
c. Student education events or curriculums relating to substance use protective factors and/or risk 
factors
d. Student leadership development activities including but not limited to: supporting youth 
leadership council focused on substance use prevention in Sauk County youth ages
12-18 years old.

6. Governing Law and Jurisdiction. The Parties agree that this Agreement shall be governed by 
Wisconsin Law.

7. Hold Harmless/Indemnification. The Department agrees to defend, indemnify and hold the Client, 
its Board, officers, employees, volunteers, and agents harmless from and against all claims and 
demands of every nature or kind on account of injury to or death of persons or
