Wisconsin Medicaid and BadgerCare Plus require the establishment of a Memorandum of Understanding (MOU) between PNCC providers and HMOs.  The MOU recognizes that the PNCC provider and the HMO have shared members and agree to cooperate and coordinate care. The MOU provides a framework for establishing a working relationship between both entities.

 

In an effort to assist you in establishing a working relationship with UnitedHealthcare Community Plan the required PNCC MOU is attached.  Please complete the PNCC Agency Information on the MOU, sign and either email me the signed document or mail the document to:
