Who is eligible / who participates in the program?
Nurse-Family Partnership is FREE to all eligible people!
Any person who:
- Is pregnant with their first child
- Is less than 29 weeks (7 months) pregnant
- Is living in Sauk County
- Qualifies for WIC or BadgerCare
What do NFP Home Visitors do?
The Nurse Home Visitor will work with the pregnant person to:
- Have a healthy pregnancy and a healthy baby
- Become a better parent
- Build a strong network of support for them and their baby
- Make the home a safe place for the baby to live and play
- Obtain referrals for healthcare, childcare, job training, and other support services available in the community
- Find ways to continue their education and develop job skills
- Set goals for their family's future and find ways to help them reach those goals
