2012 Immunization Clinic Schedule

Vaccines are free and available for children birth thru 18 years without health insurance or vaccine coverage.

Please bring your child's past immunization record with you.

A parent or guardian's written consent is required on all children under 18 years.

Immunizations are by appointment only. Please call (608)355-3290 to schedule.

VACCINES FOR CHILDREN (VFC)
Available for all children under the age of 19 years and free of charge.

DTaP- Diptheria, Tetanus, and Pertussis
Hepatitis B
Hepatitis A
Polio
HIB- Haemophilus influenzae Type B
Pneumococcal- Pneumonia
Rotavirus
MMR- Measles, Mumps, and Rubella
Varicella- Chicken Pox
HPV- Human Papilomavirus for adolescents
Tdap- Tetanus, diphtheria, and pertussis for adolescents
Meningococcal- Meningitis for adolescents
Influenza- Flu vaccine for children older than 6 months

VACCINES FOR ADULTS
Available to anyone over the age of 18. A fee for each vaccine may be charged.

Td- Tetanus and diphtheria
Hepatitis A
Hepatitis B
Pneumococcal- Pneumonia
Influenza- Flu vaccine
HPV- Human Papillomavirus for women up to age 26
Meningococcal- Meningitis vaccine
MMR- Measles, Mumps, and Rubella
Varicella- Chicken Pox vaccine
