Sauk County Home Care has been providing home care services since 1967. Sauk County Home Care is Medicare and Wisconsin State certified to provide services to all residents of Sauk County, regardless of age, race, religion or ability to pay. A registered nurse is available by telephone 24 hours a day, seven days per week.

Services that are provided include the following:

Home Health Care Aides:

Home health care aides work under the direct supervision of a registered nurse or therapist to provide services related to the patient's personal care. Aides are available to help with bathing, meals, exercise and ambulation. All home health aides are certified by the State of Wisconsin.

Occupational Therapists:

Registered occupational therapists provide treatment for improving or restoring functions impaired or lost because of illness or injury. The occupational therapy program includes training to improve self-care and activities of daily living. Family and the health-care team are taught how they can assist in the rehabilitation process. The goal of the occupational therapist is to restore as much independent function in the patient as possible.

Personal Care Worker:

The Personal Care Worker Program is coordinated by Sauk County Human Services Department.

Physical Therapists:

Physical Therapists work with the patients who have had illnesses or injuries such as fractures, strokes, arthritis, or brain injuries. The physical therapist teaches and performs medically prescribed treatment procedures and supportive activities to patients, and teaches family members to provide continued care. The goal of the physical therapist is to restore the individual to their previous level of functioning.

Skilled Nurses:

Registered nurses perform a variety of specialized and highly skilled nursing duties. They teach and advise the patient and family members, coordinate the treatment plan with other team members under the supervision of a physician. The goal of the skilled nurse is to promote independence and return the client to the previous level of functioning.

Speech Therapists:

Speech therapists work with patients who have communication problems following strokes or various brain injuries. They help patients improve by practicing identification of familiar objects, talking on the telephone, following directions and writing names. The speech therapist also works with patients who have swallowing problems caused by illness.
