Our History and Mission

Since 1871, when rolling hills and farm land were first purchased for the purpose of establishing a Sauk County "Poor Farm" our valuable history has been providing care and services for people in need. Throughout the years, whether it be managing a small pox epidemic (1904), running an intensive treatment unit for emotionally disturbed and alcoholic patients (1967), or developing an Alzheimer's unit (1996) our dedicated employees have consistently given of their combined knowledge, skills, and efforts towards developing and growing a care centered environment for Sauk County citizens. Still occupying structures that, in some limited areas, date back to the mid 1900's today we operate a modern Skilled Nursing Home (SNH) in a scenic, rural setting.

We hold as our Mission, a duty to provide Sauk County area citizens with high quality, skilled, professional, compassionate and individualized nursing, rehabilitative and supportive care and services in a modern, safe, comfortable and pleasant environment that stresses maximizing personal independence, observing and respecting the rights of individuals served, encouraging participation of family and friends and striving for achieving progressive methods and economic efficiency.

Skilled Nursing Service

Sauk County Health Care Center offers both skilled and intensive skilled nursing care. Whether you are seeking long or short term placement, in need of rehabilitation, pain management, life concluding, or special memory support care, our staff is trained and prepared to work with you and your primary physician.

Rehabilitation/Intensive Skilled Nursing Unit : Located on our second floor, this unit specializes in providing care for people in need of rehabilitation after a hospital stay or extended illness, as well as people who require such services as wound care, ostomy care, peritoneal dialysis, IV therapy, infection care or tube feeding. In addition to 24 hour a day services by a Registered Nurse and experienced Certified Nursing Assistants (CNA), we offer a full compliment of professional therapy services, including physical, occupational and speech therapy, provided by dedicated, skilled staff.

Memory Support Units: Located on the fifth floor, we have established safe, attractive, friendly, homelike surroundings for persons with memory impairment related illness. On this unit, our specially trained staff focus on individual residents abilities and needs, an active lifestyle and the involvement of family and friends. This unit allow residents to freely enjoy surroundings that include birds, fish, private rooms and a wide variety of supervised daily activities.

Respite for short term care: Sometimes when family members are the main caregivers, a respite visit to our facility can help support the caregiver and resident, by giving both individuals respite and short term breaks from their daily demands and routines. To that end, we offer respite and short term placement alternatives, with advanced notice and payment arrangements.

Activity, Social and Other Services

In addition to Twenty-four hours a day Personal Nursing Assistance provided by a team of RN'S, LPN's, and CNA's, daily services at Sauk County Health Care Center include an array of other services. We provide supportive personal help and planning from trained Social Workers : Daily large or small group as well as independent leisure activities through our Activity Department, on-site Beautician and Barber Services (appointments are required), Weekly religious programs (provided by area churches), Personal Laundry processing, Daily Housekeeping of rooms and common areas, delicious meals and snacks in accordance with special diets and professional Dietician Consultation, Dental, Optometry, Auditory or Podiatry Services, Psychiatric and Psychological Consultation, and Radiological, Pharmacy, and Laboratory services.

Volunteer Opportunities

Our team of volunteers work together to enrich the lives of residents. Whether you are interested in doing mending of residents clothing, weeding of a flower bed, transporting residents to and from activities and therapy appointments, assisting in reading and writing letters, or playing a card game, your time here will serve to reward you as much as it assists the residents you help. All of our volunteers are required to complete an application and receive training prior to beginning in their positions.
