Continuum Planning

Sauk County has undertaken an initiative to study the services offered to the elderly and disabled population in Sauk County. Termed the "continuum of care", this basket of various services is designed to be responsive to the county constituency in a fiscally responsible manner. Multiple efforts and much study has been done to move the redesign of our service model forward. A central focus of much of the discussion has been the future of the Sauk County Health Care Center, a skilled nursing facility located in rural Reedsburg, Wisconsin. The following reports highlight progress made in researching the alternatives to be offered in Sauk County's continuum of long term care, including discussion on the role of skilled nursing.
