Citizen Participation Form

Ever wanted to serve on a County commission or board in your capacity as a citizen of Sauk County? Opportunities are available to serve as citizen members for various boards and commissions that deal with issues of great diversity. Whether it be the arts and culture or health or safety, opportunities exist. The Sauk County Board of Supervisors welcomes and encourages public participation.

Use the form below to register your interest in serving as a citizen member on one of Sauk County’s commissions or boards. Submission may be made via e-mail using the button below, or printed and mailed to the address below.

Indication of your interest does not ensure appointment. Forms will be kept for two years time. As vacancies arise on boards or commissions, the County Board Chair will review requests for appointment as part of the appointment process. You will be contacted prior to appointment to ensure that your interest remains and that you are available for service.
